Personal Accountability

This form is for your growth — answer honestly, even when it's hard.

Weekly Check-In for Married Couples

"May your whole spirit, soul and body be kept blameless." — 1 Thess. 5:23

Set aside a few minutes to reflect on the past week. Brief, honest answers are enough.
Use the follow-up spaces for what needs a conversation.

Moving Toward Wholeness

Focus on what you are building, not what you are fighting.

Spiritual Foundation

Consistency in the basics creates stability in the hard things.

How many days this week did I spend time in the Word?
How many days this week did I pray intentionally?

Emotional & Relational Health

Notice what is underneath the struggle and where you need connection.

Have I been isolating from meaningful connection?
Did I handle anger, frustration, or conflict in a way I regret?
Am I carrying a burden I haven’t shared with anyone?
Have I depended on attention or affirmation to feel valuable?

Integrity & Boundaries

Integrity grows through honest choices, healthy boundaries, and bringing things into the light.

Did I dwell on thoughts or consume media that compromised my integrity?
Did I ignore or justify something I felt convicted about?
Did any interaction test or cross a relational boundary?
Was I secretive about any conversation, message, or meeting?
Have I been dishonest or kept a harmful habit or sin hidden?
Am I being fully honest in this check-in?

Physical Wellbeing & Self Care

Your body and soul are connected — how you care for one affects the other.

How well did I sleep overall?
How consistently did I nourish my body?
How often did I move my body in a way that fits my ability?
How consistently did I make room for rest and slowing down?
Did I use anything to cope or numb in an unhealthy way?

Marriage & Home

Build connection and trust through presence, prayer, and honesty.

Did I spend meaningful, undistracted time with my spouse this week?
Did I pray with my spouse this week?
Is there unresolved conflict or unspoken resentment between my spouse and me?
Have I spoken to or about my spouse in a way I would be ashamed of them hearing?
Am I attentive and responsive to my spouse’s emotional, physical, and spiritual needs?
Am I keeping anything from my spouse that would damage trust if known?

Support & Next Steps

Growth requires more than willpower — it requires the right help.

Am I currently receiving professional counseling?

You are seen. You are known. You are loved.

Rest in what God is doing in you — even when you can't see it yet.
